BAPTISM FORM (for minors)

We are excited about your child’s desire to be baptized at our church. However, it is our desire to be sensitive to you and respect the wishes of parents. Therefore, we are asking for your permission to baptize your child/children. Scott Lake Baptist Church exercises believer’s baptism. Believer’s baptism is full submersion under water, which serves as a symbolic expression of one’s faith in Christ. Believer’s baptism is only done to those who have already received Jesus as their Savior and Lord. This form must be completed before any child age 17 or under can be baptized at our church. Please complete and submit this form giving us permission for this baptism.
